Important information regarding the GRE General Test:
The GRE® General Test is changing starting August 1, 2011;the GRE® revised General Test will replace the current GRE General Test.
If you plan to take the GRE revised General Test during August – September 2011, your scores will be sent by mid-November. However, if you need your scores before November 2011, take the current test before August 2011.

Application Process:
Please note, we accept applications for Fall term only.
To guarantee full consideration please apply by January 15, 2012.
The application deadline for domestic applicants is May 15, 2012 and for international applicants
is February 15, 2012.

We strongly urge you to use the university on-line application process.
Under "Degree Admissions ", select "Domestic" or "International", review the list of required documents, and then select "Apply Online". Links are provided to view instructions and information. When completing the application, under "Select your Major" chose "PHD GRADUATE EDUCATION IN MEDICAL SCIENCES (GEMS)" from the drop down menu. The Program Code for GEMS is 20FS8060PHD. The last section at the bottom of this page describes how to rank your interest in the 5 PhD granting departments of the GEMS program. You may list up to 3 departments in order of preference with #1 as your first choice.
YOU MUST SELECT AT LEAST ONE OF THE DEPARTMENTS FROM THE LIST BELOW. ( Please note that
specifying a particular program indicates your current interest; however, students are free to choose any of the
departments for their laboratory rotations and thesis work.)

2. Completing The Application: Required Documents
Application (web version preferred on-line application process).
Graduate
Appointment form (PDF). This document is required for certain pre-matriculation Graduate College
Fellowships for which you may be eligible. Please review the guidelines for the University Fellowships and the Abraham Lincoln Graduate Fellowship. Eligible applicants may be nominated by their department of interest. As a matter of course, the Graduate Appointment form should be downloaded, filled out and mailed to the GEMS address shown below.
Personal Statement of Purpose, Relevant Experience and Goals: Please upload your personal statement in an electronic PDF file to http://dropbox.yousendit.com/GEMSFall2012. The GEMS Office has an account with yousendit.com. Submitter does not need an account or login. Please include your email address where requested so you can receive an email acknowledgement.
When submitting your personal statement:
- You may use your own stationery
- Please include your name on your statement
- The statement is not restricted to the limit stated in the Application Graduate Appointment form (see above)
- Please use the naming convention below to save your document:

Request that the Educational Testing Service send GRE and TOEFL (if
applicable) scores to the University of Illinois at Chicago. The Institution
Code for both GRE and TOEFL scores is 1851.
Additionally, select the Departmental Code for your first choice department for the relevant scores (GRE and TOEFL Department Codes differ while the Institution Code is the same). For GRE score transmission select the appropriate Departmental Code from among the following: Anatomy and Cell Biology, 0201; Biochemistry and Molecular Genetics, 0202; Microbiology and Immunology, 0212; Pathology, 0215; Pharmacology, 0216; Physiology and Biophysics, 0217. For TOEFL score transmission select the appropriate Departmental Code from among the following: Anatomy and Cell Biology, 32; Biochemistry and Molecular Genetics, 34; Microbiology and Immunology, 07; Pathology, 56; Pharmacology, 30; Physiology and Biophysics, 49.
Request that official transcripts from all postsecondary institutions
be mailed to the GEMS office.
If the transcripts are not in English you must provide the original and a certified translation. Additionally, if you have graduated from your undergraduate institution by the time of application, request an official proof of degree
be mailed to the GEMS office.
If the proof of degree is not in English you must provide the original and a certified translation. If you have not graduated at the time of application, this latter document will be required when you have graduated from your undergraduate institution.
Three letters of recommendation are required by GEMS. Recommenders should submit letters of recommendation in an electronic PDF file to http://dropbox.yousendit.com/GEMSFall2012. Submitter does not need an account or login.
Please forward the following information to your recommenders:
- Yousendit link (http://dropbox.yousendit.com/GEMSFall2012).
- Recommenders should input email address where requested and the name of the student on the subject line.
- Recommendation letters must be in PDF format, signed and written on letterhead stationery from the recommender's institution.
- Use the naming convention below to save your document:

*Please include student’s last and first name on the recommendation letter.
Application fee: domestic students: $60.00; foreign students: $60.00. With the web application you may choose to pay with a credit card at the time you submit your application on-line, otherwise send a check or money order to the University. If you defer payment you will need to send the fee to the University by check or money order. Under certain circumstances the application fee may be waived, see Application Fee Waiver Program.
